Congestive heart failure (CHF) is a leading cause of mortality globally, particularly as the population ages. Despite advancements in medical treatments and devices like pacemakers and defibrillators, outcomes for CHF remain unsatisfactory, prompting exploration of new surgical options. This special issue features contributions from experts at top medical centers, addressing critical topics such as aortic valve replacement for valvular cardiomyopathy, mitral valve repair, and myocardial revascularization in patients with ventricular dysfunction. It evaluates the advantages and disadvantages of beating heart versus on-pump coronary artery bypass surgery (CABG) and reviews clinical trials comparing CABG to percutaneous coronary interventions. The publication also discusses ongoing clinical trials in surgical CHF management, ventricular reduction and remodeling surgery, and the current status of heart transplantation. Resynchronization therapy and biventricular pacing are examined, alongside the use of ventricular assist devices for destination therapy and as a bridge to transplantation or recovery. Updates on the Jarvik 2000 ventricular assist device and the AbioCor total artificial heart are included, as well as innovative approaches like gene therapy for coronary artery disease, therapeutic angiogenesis, transmyocardial laser revascularization, and cardiac xenotransplantation.
